My brother and I were working in a small tool shop. Another guy worked there was quite a drinker. He was working on an old lathe that used a 1 in chuck key - with a handle about 2 feet long. He forgot he had it in the chuck, and put it in reverse too. The chuck key went right past my brothers head and thru a glass window. My brother just looked around and walked out for the day.
