While refusing medical treatment is the right of any person; I feel this young man has shown other signs that will outweigh any rights he may have. He does seem to me to be either scared of what the doctor will tell him in fear of facing the facts; or suicidal. In either case the police should be called and let them force treatment on him. At least the police can force him into a 72 hr evaluation to see if he is of sound mind to make a decision like this.
As far as the wife being scared he will leave if treatment is forced; where will she be if he dies. In the same boat I would assume. At least if she has him committed he does have a chance to get his head on straight and forgive her. This will not be possible if she lets him die. And we need to add the fact that she will have to live with that decision the rest of her life.
I seem to remember others in your family having bad depression problems not to long ago so you may know just how he feels.
