There could be a hundred different things wrong with that H. If it only tries to move in 5th gear somehow power isn't getting to the lower shaft in the transmission. Does the rear PTO work? or the belly pump, or the belt pullley? Could be something as cheap as a sheared key on the lower constant mesh gear or it could be stripped teeth on those gears and any number of other gears.

The bottom end of the shift lever has worn out and allowed the lever to jump out of the shifting rails. Pull the lever out, reach down in the hole with a large screwdriver or pry bar, and line up the notches in the shift rails. You can then put the lever back in, and it should be good enough to at least test drive the tractor, but shift it very slowly. The lever itself will need repaired, but you can address that issue later.

The lever comes out by removing a pin behind plugs in the shift tower. Try not to give the impression that it is fixed. It will still require repair. Fair value should include the needed work, it will happen again. search here for a ton of data on how to fix.
