As stated below, there are two controls and three arms. One control moves a pair of arms, and one control moves an arm on the left side.

It sounds like the belly blade was hooked up to both controls so you could lift and lower each side independently. Neat. Works like a "real" road grader, except that it doesn't work :(

Get yourself a can of PB Blaster or Kroil and start squirting all the pivot points for that left lever. It may be that only the linkage is rusted up and there is nothing wrong with the cylinder.
