The size being refers to is the outer diameter of the flange.
The crankshaft needs to be in the correct location to get the bearing to roll into place. You only have a few thousands of an inch clearance there. You will need to move the shaft forward or back to get it in the correct place.
The C-221 engines in the 460 used the 3 3/4" flange bearing but I have always been able to used the wider flange one when overhauling them. You may want to check for sure that your block has been machined enough to accept the wider flange. I think all the blocks were machined to be able to use the wider bearing but it doesn't hurt to check. The 560 used the same block and they came with the wider bearing.
