I've had plenty of precoms blow out esp when a 3/4" thread was used, the fix for that was tap the hole to take the 7/8 chamber instead, that was done with the head in situ,if yours is 7/8 you will have to use a helicoil,Cat did a helicoil kit for the job, there is a picture of the kit and a bit of info at the link below.

Not very common..... but not unheard of. Loss of threads due to corrosion or galled threads from last removal. These are normally very tight, requiring 225 ft. lbs of tightening torque and tend to weld themselves together if neversieze is not used on installation. My guess is they were not tightened fully when last installed....maybe due to known thread problem.

Forgot to mention the fix....
Cat had a heli-coil thread insert and tooling for repair. Sorry I can't tell you the size as it's all in Cat part numbers. The drill size was 1-1/4 in. and the tap is a #8F5578 along with assorted guides, sleeve and prewinder.

I've also head of the stripped thread being welded up and re-machined.

I think I would check the pump timing. Best go to ACMOC or ACME site. People there will tell you exactly what to do.
