Report it to your insurance company. You made an appointment, you appeared when you were supposed to and your doctor failed to take care of you within a reasonable length of time. It's one thing if were at fault for missing an appointment, quite another if she was at fault.
If insurance refuses to pay, and you refuse to pay, the worst she can do is ding your credit report. If you don't report it to insurance, you're a participant to fraud.
