I had the same issues, and I went with "semi-custom" orthotics.
Any good shoe store will have a machine that measures pressure points in your feet. You stand on a pad, it measures the pressure in various areas, and recommends a pre-fab insert.
$60-ish for a pair, but in my case the relief was instantaneous.
This was AFTER blowing $200 on a "profesionally" fitted pair of Redwings that actually made the problem worse. I now wear Carhartts (who knows who actually makes them) at half the price with the inserts.
