My 7.3 in my F250 go so the timing-advance no longer worked at all. Even with NO advance, it did not give the symptoms you're getting. My Dodge with the 5.9 Cummins had the same problem. Having no advance really shows up when the engine is cold. Engine will smoke, skip, and break up - especially when you rev it up until it gets warmed up. Then run pretty much, OK.
When's the last time your engine had some new injector nozzles? You can sometimes find new ones for $10 each and they rarely last past 200K miles. When those pintle injectors get worn, they can stick and cause a lot of smoke when they do.
