Mike, I think I may have figured out how I screwed up. To make a long story short and easy to understand here is what may have happened.
When I unhooked the trailer the truck was empty and I may have put too much blocking under the jack. When I hooked the truck back up to trailer, I had weight in bed of truck. Cranking the jack back down I thought all was well when in fact I may have closed the hitch and the ball wasn't fully in place.
Lesson learned. Check to see if ball is all the way in place. Bet there isn't a thing wrong with hitch or ball.
