The major cause of acidents/mistakes is the belief that nothing is going to go wrong with what you are doing. Some sort of belif in personal imortality because of one's own skills or abilities etc.
Often happen because of experience, lack of perception caused by hurry,weariness or loss of concentration by bordom or worry.
