I had a 93 Ford pickup with this problem. I had the fluid changed with the flush method at a quick lube. It fixed it for a while then it came back so I changed it again and added anti shudder additive. It eventually came back so I had the torque convertor replaced. Problem solved. It wasn't all that expensive but I had a Ford mechanic that did it on a weekend. About $300, parts and labor.
