Do you have the (standard) transmission-driven PTO, or the (optional) constant-running PTO?
(Either way, the PTO disconnect clutch is the same.)
I have a 424 diesel with the continuous-running PTO. A couple of years ago, it started giving problems much as you describe.
Turned out the casting in which the PTO lever's shaft pivots was cracked, and would deflect, allowing the PTO disconnect clutch to easily "jump out of gear".
In the photo, the pen points to the part, 704 392 R91.
