I was following my uncle on the highway this April, and got to watch the whole trailer come undone, a tandem axle tilt bed. He was fixing to go get it all ready to go, including fixing the safety chains. He was taking it to a shop just a few miles up the road to do just that when it came undone... snapped the lone chain on there and it went around his truck the same as some of the tire examples here.
He and I were lucky... popped a tire on the truck getting it stopped but it was a small price to pay to get the mess off the road. Otherwise there was no damage.
