Years ago back on the farm, we just took a plate of steel, about 1/2" if I remember, long enough to fit between the raild and about a foot wide. The local garage could weld this in place without removing the bed. We cut a hole in the bed and the plate for the ball, and welded the nut in place. We left it in place all the time.
We always mounted the ball just forward of the rear axle.
