If the pressure gage takes off immediately, you have a bad head gasket. If it runs for a minute and the pressure rises only slightly, the head gasket is good and you can look elsewhere.
If you put the pressure tester on the radiator after running the engine enough to stabilize the temperature, any rise in pressure is a good indication of combustion gasses entering the cooling system.
The fact that the heat comes and goes with the cooling system full is almost always an indication of a combustion leak into the cooling system.
