MarkB gave the explanation as I understand it. The claim is that a well funded company like Google or Microsoft could pay to have their traffic delivered before anyone else's, causing companies who don't pay extra to suffer.
I assume the ones who would actually suffer are things like peer-to-peer file sharing (i.e. music pirates). The talk of companies using it to crush competitors sounds more like scare tactics to me. Voice over IP is another issue though because it is in the phone company's interest to move that traffic as quickly as possible.
