I would bet on the pin hole in sleeve. O-rings will of course be shot as well. Over the years I have seen more than one of those engines with oil that is foamy in radiator and was from pin hole. Some of them the owner replaced oil cooler first but that didn't solve problem. If you have a leaky oil cooler the oil in radiator will be plain old black and whereas a pin hole will be frothy oily crud. Oil coolers are expensive but can be tested as you will need to take it off anyway to clean it out good. We are talking D-360 aren't we.
