The most common cause of shoelace knots coming undone is that they are being tied incorrectly as a Granny Knot. As mentioned above, the tell-tale sign is that the bow sits along the shoe instead of across. To fix it, tie your first knot the opposite way. In other words, if you normally tie "left-over-right", tie it "right-over-left", then complete your knot as normal.
